| Abstract: | O presente relatório evidencia o percurso desenvolvido em Enfermagem de Saúde Infantil e Pediátrica, com enfoque na aquisição de competências especializadas e de nível de mestrado. Desenvolveu-se o projeto “Cuidado Ativo: A Jornada Terapêutica do Adolescente”, centrado na promoção da literacia em saúde, do autocuidado e da capacitação de adolescentes com doença crónica. A fundamentação integrou o Modelo de Promoção da Saúde de Nola Pender, a Teoria da Aprendizagem Experiencial de Kolb e o Modelo de Cuidados Centrados na Criança. Recorreu-se ao Design Thinking para identificar necessidades e desenvolver intervenções educativas em diferentes contextos. Os resultados evidenciam melhorias na autonomia e participação nos cuidados, reforçando o papel do enfermeiro especialista na promoção da saúde e humanização dos cuidados; Active care: the therapeutic journey of the adolescent - Abstract: This report presents the learning pathway developed in Pediatric and Child Health Nursing, focusing on the acquisition of specialist and master’s level competencies. The project “Active Care: The Adolescent Therapeutic Journey” was developed, aiming to promote health literacy, self-care, and empowerment in adolescents with chronic conditions. The theoretical framework was based on Nola Pender’s Health Promotion Model, Kolb’s Experiential Learning Theory, and the Child-Centered Care Model. Design Thinking was used as a methodological approach to identify needs and develop educational interventions across different care settings. The results highlight improvements in autonomy and active participation in care, reinforcing the role of the specialist nurse in health promotion and the humanization of pediatric care. |
