O ciclo PDCA no sistema de controlo interno em ambiente hospitalar no SNS

Abstract

O Sistema de Saúde Português é alvo de constante validação por parte da opinião pública, havendo uma monitorização transversal sobre toda a prestação de serviços de saúde, sendo o Ministério da Saúde e toda a hierarquia de locais de prestação os responsáveis das suas estruturas e da oferta que disponibilizam à população. O modelo hospitalar é o modelo mais complexo dentro da hierarquia de locais, nomeadamente devido à prestação de cuidados especializados e, alguns, de alta diferenciação e evidência cientifica. Perante tal complexidade e resposta especializada à necessidade de cuidados em saúde da população, torna-se imperativa uma relevância funcional em termos de controlo interno, potenciando a garantia do cumprimento legislativo, regulamentar, ético e científico na prestação de todos os cuidados. Importa que o efetivo controlo interno esteja presente dentro da instituição hospitalar, de forma transversal e permanente, em todas as atividades, despoletando um ambiente de controlo que promova uma avaliação de riscos, onde a sua resolução passa por atividades de controlo, as quais são informadas e comunicadas a toda a organização e, finalmente, carecem de monitorização constante. O ciclo PDCA (Planear, Fazer, Verificar, Agir) é uma metodologia estruturada e fundamentada que, de forma entrosada e disseminada em todo o ambiente hospitalar, promove que o controlo interno seja potenciado e alcance um patamar satisfatório, que garanta aos Conselhos de Administração dos Hospitais e a toda a população uma prestação de cuidados de qualidade enquadrada numa estrutura funcional em constante melhoria. A análise desta correlação foi materializada num questionário remetido a todos os Hospitais, de onde resultaram 277 respostas válidas, as quais evidenciam um paralelismo significativo entre o ciclo PDCA e o Controlo Interno em ambiente hospitalar no SNS; Abstract: The PDCA cycle in the internal control system in hospitals in the National Health Service The Portuguese Health System is a constant target of validation by the public opinion, having a cross monitoring on all the provision of health services, being the Ministry of Health and all the hierarchy of places, those responsible for its structures and the offer available to the population. The hospital model is the most complex model within the places hierarchy mainly because of the provision of specialized care, and in some cases, the high differentiation and scientific evidence. Because of this complexity and specialized response to the need for care in health of the population, it becomes mandatory that a functional relevance in terms of internal control, enhancing the guarantee of compliance on legislative, regulatory, ethical and scientific matters in the providing of care. It is important that effective internal control is undertaken within all of the hospitals operations , transversally and permanently, triggering a control environment that promotes risk assessment, where their resolution is found in control activities, which are reported and communicated throughout the organization and, ultimately, require constant monitoring. The PDCA cycle (Plan, Do, Check, Act) is a structured and reasoned methodology which, mingled and disseminated throughout the hospital environment, promotes that internal control is enhanced and reaches a satisfactory level that ensures that the Hospitals’ Board of Directors and the entire population have the provision of quality care, framed in a functional structure in constant improvement. The analysis of this correlation was materialized on a questionnaire sent to all Hospitals, which resulted in 277 valid responses, which show a significant parallel between the PDCA cycle and the Internal Control in hospitals in the National Health Service.
