Abstract: | O utente com alteração de saúde mental e psiquiátrica surge no serviço de urgência geral como "um entre muitos". Porém, a especificidade desta área de intervenção e a inexistência de profissionais com conhecimentos específicos que possibilitem uma intervenção cuidada, atempada e eficaz tem influência na qualidade dos cuidados prestados, acrescendo os custos em saúde, pela dificuldade de gestão dos recursos existentes. Foi com base em toda esta problemática e na ineficácia de intervenção que existia face ao doente com alteração de saúde mental e psiquiátrica no serviço de urgência geral do Hospital José Joaquim Fernandes, que me propus desenvolver nesta área um projecto de intervenção específico e simultaneamente abrangente. O projecto teve início em Março de 2007 e o presente relatório refere-se ao triénio 2007 a 2009. O objectivo deste relatório foi descrever o potencial de intervenção existente no serviço de urgência geral, as intervenções efectuadas, os resultados e ganhos atingidos em saúde. Numa primeira parte foi feita uma análise e caracterização dos doentes admitidos e triados pelos fluxogramas potencialmente detentores de alterações de saúde mental e psiquiátricas; na segunda parte foi feita a caracterização dos utentes com os quais efectuei intervenções e por fim uma análise dos resultados atingidos com essas intervenções. Os resultados obtidos nestes três anos de desenvolvimento do projecto conseguiram-se ganhos importantes em saúde: a equipa multiprofissional do serviço de urgência mais desperta e motivada para a problemática deste tipo de Utentes, uma melhor qualidade dos cuidados prestados, uma utilização e gestão dos recursos existentes nesta área para resolução atempada das problemáticas existentes e uma redução de custos com a deslocação por vezes desnecessária para um hospital central; ABSTRACT: The patient with mental and psychiatric disturbance appears in the general emergency room as "one among many others". However, the specificity of this intervention area and the existence of professionals with skills which allow a careful, timely and effective intervention affects the quality of the care provided and increases health costs, due to difficulties in the management of existing resources. Based on this issue and on the intervention inefficiency that used to prevail towards the patient in Hospital José Joaquim Fernandes, I decided to develop an intervention project, both specific and comprehensive. The Project began in March, 2007 and the present report refers to the triennium 2007/2009. The goal of this report is to describe the existing intervention potential in the general emergency room, the interventions carried out, the results and the gains. Firstly, an analysis and a portrait of potentially mental and psychiatric disturbance holders, allowed and screened by the flowcharts, was carried out. Secondly, a portrait of patients with whom I had interventions was made and, finally, an analysis of results reached by those interventions is presented. The results obtained in the last three years of project development show important health gains: the emergency room multiprofessional team is more alert and motivated to this type of patients issue; the use and management of existing resources in this area, aiming at a timely resolution of troubles and a reduction of costs (obtained with the unnecessary transfer of patients to a central hospital), is now more effective. |
