Abstract: | O presente estudo pretendia avaliar variações no perfil de ajustamento psicossocial à doença e ao tratamento, de acordo com o diagnóstico e a modalidade terapêutica. Investigou-se também o papel das representações de doença no processo de ajustamento emocional e a adaptação funcional a médio prazo. O estudo adoptou o Modelo de auto-regulação de H. Leventhal, Meyer e Nerenz (1980) sobre o ajustamento à doença. Este estudo foi realizado numa amostra portuguesa de 124 pacientes com enfarte agudo do miocárdio (EAM) e angina instável (AI), 96 homens e 28 mulheres, com idades compreendidas entre os 50 e os 86 anos, obtida em três hospitais, dois da região de Lisboa e Vale do Tejo e um da Região do Alentejo. É um estudo de coorte, do tipo longitudinal correlacionai-diferencial sendo os doentes avaliados em dois momentos: na altura da alta hospitalar e três meses após a alta. Este estudo orienta-se pelos seguintes objectivos: identificar o perfil da adaptação psicossocial nos pacientes com EAM e AI, de acordo com o tipo de terapêutica (farmacológica, angioplastia e cirurgia aorto-coronária por bypass), na alta e aos 3 meses; descrever e comparar as representações de doença, em pacientes com EAM e AI e de acordo com a modalidade terapêutica (farmacológica, angioplastia e cirurgia aorto-coronária por bypass); analisar o papel das representações de doença na alta, como preditores de adaptação psicossocial após a alta (aos 3 meses); identificar o papel da satisfação dos pacientes com a informação comunicada pelos profissionais de saúde, e os cuidados de saúde em geral, na adaptação psicossocial ao EAM e AI; explorara importância das variáveis sócio-demográficas e clínicas, na adaptação psicossocial pós-EAM e AI. Os instrumentos de avaliação utilizados foram as versões portuguesas do Sickness Impact Profile – SIP, para avaliara adaptação funcional; do Hospital Anxiety and Depression Scale – HADS, para avaliara adaptação emocional; e do Rlness Perception Questionnaire Revised – IPQ-R, para avaliar as representações de doença. Apesar de não ser objectivo deste estudo, contribuímos para a validação portuguesa do IPQ-R e do HADS, pelo que são apresentadas as características psicométricas dos instrumentos na metodologia deste estudo. as hipóteses testadas: as hipóteses 1 e 2 previam que a adaptação psicossocial à
doença no momento da alta variasse consoante o grupo clínico/diagnóstico, enfarte
agudo do miocárdio (EAM) ou angina instável (AI) e consoante os tipos de
modalidade terapêutica. Os dados obtidos na sua generalidade não comprovaram a
hipótese 1, quer ao nível da adaptação emocional, quer ao nível das representações
acerca da doença, quer da satisfação com a informação comunicada ao paciente, e
com a satisfação global dos cuidados prestados no contexto hospitalar. O gênero
parece ser uma variável sócio-demográfica importante na adaptação emocional,
estando as mulheres mais deprimidas do que os homens na alta. Os níveis de
ansiedade variam com o diagnóstico nas mulheres, apresentando as mulheres pós
enfarte níveis de ansiedade mais graves. Ao nível das representações de doença, as
mulheres possuem cognições mais negativas do que os homens na duração,
consequências, coerência, duração cíclica e representação emocional. Em relação à
satisfação com a comunicação-informação, e os cuidados globais, os resultados
indicam que estas variáveis são independentes do diagnóstico mas existe uma
variação com o gênero, estando as mulheres mais insatisfeitas. A hipótese 2 previa
que a adaptação psicossocial à doença no momento da alta variasse com o tipo de
terapêutica: farmacológica, angioplastia e/ou cirurgia aorto-coronária por bypass. Os
dados comprovam parcialmente esta hipótese, relativamente à variação da adaptação
emocional com a modalidade terapêutica, da variação do nível da satisfação com a
informação comunicada ao paciente, e da satisfação global com os cuidados prestados
no contexto hospitalar, embora estas variações se situem ao nível do gênero e do
grupo etário. Nas mulheres mais novas, a angioplastia associa-se a maior depressão e
nas mais velhas a cirurgia bypass, enquanto nos homens mais velhos, a depressão
mais elevada está associada à farmacoterapia. As representações cognitivas, no
momento da alta, não variam com o tipo de terapêutica. Ao nível da satisfação com a
informação comunicada ao paciente, os resultados revelaram uma influência da
interacção da idade e da terapêutica, com os pacientes mais velhos submetidos a
bypass, e os mais novos sob tratamento farmacológico a apresentarem maior
insatisfação.
/ABSTRACT - The present study aimed at examining the psychosocial profile of adjustment to illness and treatment, according to the diagnoses and treatment mode. The role of illness representations in emotional and functional adjustment at 3 months was also investigated. The study adopted the modal by H. Leventhal, Meyer and Nerenz (1980) of self-regulation on illness adjustment. The study includes a Portuguese sample of 124 patients with acute myocardial infarction (AMI) and unstable angina (UA), 96 men and. 28 women, with ages between 50 and 86 years, collected in three hospitals, two of the Lisbon and. Tagus Valley region, and one of the Alentejo region. It is a cohort study with a longitudinal correlational and group comparison design, with the patients being evaluated at two moments: at the time of discharge from the hospital and three months after the discharge. This study is guided by the following objectives: to identify the psychosocial adjustment profile of patients with AMI and UA, according to the type of treatment (pharmacological, angioplasty and bypass aorto-coronary surgery), at the time of discharge, and three months later; to describe and compare illness representations, in patients with AMI and UA, and according to the type of treatment modality (pharmacological, angioplasty and bypass aorto-coronary surgery); to examine the role of illness representations at the time of discharge as predictors of psychosocial adjustment after the discharge (3 months later); to study the role of patient satisfaction with the information given by health professional, and with the tare provided, in the psychosocial adjustment to AMI and UA; to investigate the impact of socio-demographic and clinical variables in psychosocial adjustment post-AMI and UA. The assessment instruments used were the Portuguese versions of the Sickness Impact Profile – SIP, to assess functional limitations; of the Hospital Anxiety and Depression Scale — HADS, to assess emotional adjustment; and of the Illness Perception Questionnaire Revised — IPQ-R, to evaluate, illness representations. Although it was not a goal of this study, we have contributed to the Portuguese validation of the IPQ-R and the HADS, and hence, their psychometrics characteristics are presented in the methodological section of this
study. Six hypotheses were tested: Hypotheses 1 and 2 predicted that psychosocial adjustment to illness at the time of discharge would vary according to the clinical group/ diagnosis, acute myocardial infarction (AMI) or unstable angina (UA) and according to the type of treatment. Overall, the data did not support hypothesis 1, in terms of emotional adjustment, illness representations, and satisfaction with information communicated to the patient ant with the care provided in the hospital context. Gender seems to be an important socio-demographic variable in emotional adjustment, with women reporting more depression than men at discharge. Among women, the levels of anxiety vary according to diagnosis; with women post AMI presenting more severe anxiety than those with UA. Regarding to illness representations, women reported more negative illness representations than men, in terms of duration, consequences, coherence, cyclical duration and emotional representation. Regarding to satisfaction with communication-information and global care, results show that these variables are independent of the diagnosis, but there is a variation with gender, with women being less satisfied than men. Hypothesis 2 predicted that psychosocial adjustment to illness at the time of discharge would vary according to the type of treatment: pharmacological, angioplasty or bypass aorto-coronary surgery. The results partially supported this hypothesis, concerning the variation of emotional adjustment with treatment modality and the variation of the level of satisfaction with the information communicated to the patient, and of global satisfaction with care, although these variations occur at the level of gender and age groups. Among younger women, angioplasty is associated with greater depression, and among older women this happens for bypass surgery, whereas in older men higher depression is associated with pharmacotherapy. Cognitive representations at the time of discharge, do not vary according to the type of treatment. With regard to satisfaction with the information communicated to the patient, the results revealed an influence of the interaction of age and treatment, with older patients submitted to bypass, and younger patients undergoing pharmacological treatment showing higher dissatisfaction. Hypothesis 3 predicted that psychosocial adjustment would vary over time, from the time of discharge to three months later, with that variation depending upon the clinical group and treatment. |
