Abstract: This chapter addresses the development of self-care competence in older adults living with a bowel elimination ostomy, a population that frequently experiences complex physical, psychological and social adaptation demands. Drawing on current nursing evidence and structured self-management approaches, the chapter outlines key domains of self-care competence relevant to ostomy care, including stoma and peristomal skin management, appliance handling, complication recognition and early response, dietary and hydration management, medication considerations, and strategies to support autonomy and confidence in daily activities. Particular emphasis is placed on person-centred education, skills training, and ongoing follow-up tailored to health literacy, cognitive and functional status, and available social support. The chapter also highlights the role of nurses in assessment, individualised care planning and coaching, as well as the importance of monitoring outcomes such as self-efficacy, quality of life, and preventable complications. The synthesis aims to inform clinical practice and support the design of nursing interventions that strengthen self-care competence and facilitate successful long-term adaptation to living with an ostomy
